Forms that can be transmitted over the Internet
At Clinique Dentaire Dr. Wayne Dionne, electronic transmission (via the Internet) of your dental insurance forms is offered at no extra charge. We usually receive a reply immediately after transmission as to the amount you will be granted for your dental treatment. All you have to do is pay the deductible and your co-insurance. In most cases, the portion not covered by your insurance is a percentage of the total amount. Usually, this percentage is 20%.
Forms that cannot be transmitted over the Internet
Not all forms can be submitted electronically. When they are, they are completed free of charge and delivered to you personally. You will be responsible for ensuring that these forms are delivered correctly, and for paying the dentist's fees.
What you need to know
We want to remind you that insurance coverage varies, depending on the agreement signed between the insurance company and your employer. It is illegal to overcharge your insurance or not to claim the difference. It is therefore your responsibility to find out about your coverage and the maximum amount allowed per year.
